I think Jack's nose started to bleed because he probably didn't want anybody he knew to find out and tell his mom and he might have been thinking about what his mother was going to do if she were to find out. I also think that Jack didn't want to disappoint Mr.Spizz because he might be mad and tell the cops about Jack's dad's run way so he may have wanted to hurry up and not make Mr.Spizz mad.
I think Jack's nose started to bleed again because Miss Volker probably did the surgery wrong and its starting to bleed again and it's probably worse then it was before Miss Volker did the surgery and I think that his nose started to bleed again because he was so worried about his mom finding out.
I think that it is from the surgery Miss.Volker performed because if you remember she said that this would happen. In chapter 11 after the surgery she said,"I'm all done. It will hurt a little tomorrow and my bleed a bit during the scabbing stage, but you should be fine."
